Photo Release Form

PHOTOGRAPH RELEASE FORM

I do hereby give permission to the Family Resource Center to use my photograph or my child’s photograph in all forms and media for purposes of furthering the cause of quality early childhood education.  I waive the right to inspect or approve the finished versions, including all written copy that may be created in connection therewith.  I agree that this release shall be binding on me, my legal representatives, heirs and assigns.  I am of full age and have read this release and am fully familiar with its contents.

I also grant the same permissions to the FRC’s partners such as the Eau Claire Community Foundation, public libraries in Eau Claire County, and Too Small to Fail to use my photograph or my child’s photograph.
